Hacienda Heights AYSO Region 23 Parent Agreement We want to thank you and your family for choosing to participate on one of our Hacienda Heights AYSO Region 23 Fall Teams for the 2016 season. As a parent, there are many responsibilities that you have. One thing we ask is that you bring your child to all practices and games on a regular basis and that you arrive at the time the coach requests. Getting your child to all practices and games ON TIME depends mostly on you. Your commitment level is as crucial to the success of the team as it is the players. Remember, you are the ultimate role model for your child. By signing this form, you agree that you, and any family members or other guests invited by you, will follow the guidelines of Hacienda Heights Region 23 as stated below: SPECTATORS:       

Failure to comply WILL result in termination of your rights to attend any further games this season. The use of tobacco on ANY soccer field where children are present is expressly forbidden. Animals are NOT allowed on ANY AYSO soccer field at ANY time. You must sit on the same side of the playing field as the coach of your child’s team.

You also agree to practice proper behavior at ALL games, on ALL fields, towards ALL referees, parents and players. Failure to adhere to these guidelines may result in an ejection from the field and/or Regional Board review which can lead to YOUR suspension and/or that of your child. CONSEQUENCES: 
