2017 Scholarship Information Description: Hazel Green Funeral Home recognizes the importance of further study to achieve career goals, and this scholarship is available to students who will attend a trade school, certificate program, community college or university in the fall of 2017.

Purpose: This scholarship provides a $1000 award to a graduating senior. The recipient will be selected by an independent committee on behalf of Hazel Green Funeral Home based on fulfilling these criteria: • • • • •

Demonstrates a career goal that will be achieved through further education. Demonstrates financial need to reach this goal, and shows appreciation of financial assistance. Has a 3.0 or greater GPA Must not be the recipient of a substantial scholarship or need-based grant. Receives a positive letter of recommendation from guidance counselor, teacher, or employer.

Complete the scholarship application form (available in the guidance office or on our website at www.hazelgreenfuneralhome.com). Include one letter of recommendation from a guidance counselor, teacher, or employer. 1n 500 words or less, explain your career goals and how you plan to achieve them. Also explain your financial need of this award money and how it will help you reach these career goals.
