Important High School Athletics Information REQUIRED FORMS As an athletic team member, your child must have completed the follow forms prior to participating in any athletic event (practice or games). Please email scanned copies to [email protected] or hand into the high school receptionist. 1. Physical Evaluation and Physical Assessment Form a. Assessment MUST be completed and signed by a doctor 2. Impact Testing (Mandatory for all contact sports) – more info to come soon 3. Concussion and Traumatic Brain Injury Form 4. Sudden Cardiac Arrest Form 5. TAPPS Parent and Student Notification Steroid Use Agreement Form 6. TAPPS Student Acknowledgement of Rules Form 7. TAPPS Transfer Form a. If you are transferring to The Village School for the 2015-2016 academic school year and have already started 9th grade at a previous school, you will need to complete our TAPPS Transfer Form to be eligible for Varsity Athletic participation 8. Parent and Student Athletic Consent Form 9. Athletic Code of Conduct Handbook Acknowledgement Form a. (Updated and released Summer 2015) All forms will be kept on file in the athletic offices, and will ensure each students opportunity to participate.
