This survey consists of case notes, questionnaires and relevant technical material (e.g. unstained slides). Please follow instructions given with each case. You will only receive Instructions to participants and slides for the modules you have enrolled in. Please check your enrolment confirmation form, if you are unsure of your enrolment details. All samples are tested for homogeneity before distribution by examining sections at frequent regular intervals throughout the full set of slides. Participants should treat survey material in the same manner as routine samples within their laboratory. All material submitted to the RCPAQAP Anatomical Pathology must be clearly labelled with your participant number. Participants must submit ALL questionnaire responses using the RCPAQAP website at https://anatomical.rcpaqap.com.au If you have previously been registered to use the RCPAQAP Anatomical Pathology website you will NOT be required to re-register. However if you are not registered, this needs to be done prior to result entry. Please register immediately to use the QAP Anatomical Pathology website for 2014 modules. Participants are required to enter a unique user ID. Following successful registration RCPAQAP Anatomical Pathology will validate the registration and an email containing the password will be sent to the registered email address. Validation of registrations will only occur during business hours.
Survey closes at midnight Australian Eastern Standard (or summer) Time (AEST).
It is suggested that a printed copy of your responses to the questionnaire is retained for future reference. Please contact the RCPAQAP Anatomical Pathology for assistance if required.

Assessment criteria The RCPAQAP will assess the immunohistochemical labelling according to the following criteria:
Intensity of true positivity is of reasonable strength
Absence of background staining
Sensitivity – all target tissues labelled
Localisation – only target antigenic sites labelled
Chromogen character – crisp and distinct
Counterstain quality – complementary not obscuring
Absence of artefacts Five members of the Immunohistochemistry Committee independently score each slide. Each member gives a total mark out of 5, which is then reported as an average. Assessment result

Normal Tonsil and Gastrointestinal tumour
Macroscopy
A composite block of normal tonsil and gastrointestinal tumour. Staining for Ki-67 and C-Kit (CD117)
Instructions Three (3) unstained sections are provided. The tissue has been fixed in 10% formalin for 48 hrs, processed and cut at 4µ onto charged slides onto (FLEX IHC) and dried at 60°C for 30 minutes. The first antibody for assessment is Ki-67. Any participant not routinely using this antibody should tick the “Not applicable” box in the relevant section of the submission form. This is to ensure that participation in this exercise is recorded. The second antibody for assessment is C-Kit (CD117). Any participant not routinely using this antibody should tick the “Not applicable” box in the relevant section of the submission form. This is to ensure that participation in this exercise is recorded. Apply your Ki-67 and C-Kit (CD117) antibody to the provided sections, and submit your stained slides (clearly labelled with your participant number) to the RCPAQAPQAP for assessment. The RCPAQAP Anatomical only requires the submission of one slide for each antibody to be assessed in this exercise. The RCPAQAP will not assess any controls for this exercise. Please note: Technical material must be received at the RCPAQAP office by the closing date. Please mark for attention of the RCPAQAP Anatomical Pathology. All material submitted must be permanently, legibly and correctly identified with your enrolment number. We will not assess any material submitted that is not correctly identified or where the identification is illegible.
