Items must be postmarked within 15 days of purchase of product to be valid. This offer cannot be combined with any other offers. IRIS Inc. is not responsible for lost or misdirected mail. Requests from P.O. boxes will not be accepted, and requests with invalid or undeliverable mailing addresses will not be honored.
