AYSO REGION 106 REFUND POLICY & PLAYER DROP FORM SPRING RECREATION Players that drop from our league may receive a refund per the following guidelines and procedures: Guidelines: 1. Any player, who withdraws prior to March 1st, of the year in which the season begins, will result in a full refund of the Registration fee, providing that the player/parent submits a request for a refund by completing the region 106 Refund Policy & Player Drop Form. 2.

Any player who withdraws on or after March 1st, of the year in which the season begins ,and prior to opening day will result in a full refund, less the cost of the AYSO National Registration Fee, provided that the player/parent submits a request for a refund by completing the Region 106 Refund Policy & Player Drop Form. The cost of the uniform will also be deducted from any refund unless the uniform is returned unused.

Any player who withdraws on or after participating in the first game of the season or anytime thereafter shall not be entitled to a refund.

Procedure: 1. Parent: The parent/guardian requesting the drop must obtain a Player Drop Form, complete the “Parent” section below, sign and return to the coach. The form can be obtained from the Region 106 website. (Prior to being assigned to a team, Parent must forward the completed form to the Regional Registrar. (See address below) 2.

Coach: The team coach must send or hand-deliver the Player Drop Form with the Player Registration Form to their Division Coordinator.

Coordinator: The Division Coordinator must insure that the form is completely filled out and sent or hand-delivered a copy of the Player Drop Form and Registration Form to the league Registrar.

4.

Registrar: The Registrar will notify the league Treasurer of the amount of the refund and where the send the refund. The Regional Commissioner will be notified of every player drop
