*'t*Late Spring 2Ot7 Registration Form for U6*** Late Registrations are due by January 15,20L7 Registration lnstructions Note: Please use this form for Under 6 registration only. Players born after July 3t, 2OL0 should use this form. Those players that are older than U6 can obtain a registration from our website: www.bo.onecountvsoccer.com. Late registrations are due by Jan. 15,2017. The cost for late registration is $75. For a ny UG registration questions please ernail: [email protected]. For any other questions please contact Adele Nichols at: 859-525-1070. Please fill out this form completely and return it to the address listed on the bottom of this form. Make sure to include a check made out to the BCYSA for SZ5 for each player. The BCYSA will charge a S25 for any returned check. First time players must inctude a copy of their birth certificate to verify age" Player Placement Since this is Late Registration all registrations at this point are accepted on a first come first serve basis and there is no guarantee of placement on a particular team or any team at all. No requests. tf the child cannot be placed on any team, the check will be returned.
There will be no other reasons for refunds after Feb 1.. We ask that all players be at least 3 years old by July 3L,2016. Registrations will not be accepted over the phone or through the internet. Please note that in recreational soccer, coaches cannot request who is placed on their teams.
