____Current 1508 Evaluation or Re-evaluation/Waiver ____Parental Notification Letter For Transition aged students – ____Documentation that Student was invited to the meeting ____Parental Permission to Invite Agencies to IEP Meetings (found on Parent Notification Letter) Age of Majority Letters (age 16, turning 17 before next IEP Review Date, & no later than 1 year prior to 18th birthday) These letters are sent once and kept in the IEP folder until student exits the school system. One letter is sent to parents and one letter is sent to the student.

For Transition Students (age 15 turning 16 during the school year and older) ____Transition Assessments (formal and informal) used to determine Post Secondary Goals and to document Current Levels of Performance for goals targeted for Transition. ____Transition Interest Inventories used to determine Post Secondary Goals and to document Current Levels of Performance for goals targeted for Transition. ____Copy of FAX inviting Agencies to IEP Meeting (if you had parental/student permission to invite) ____Copy of Agency Contact Information provided to student/parents at the IEP Meeting (highlight specific ones which are pertinent to the student’s needs and Post Secondary Goals) ____Copy of Community Based Instruction Permission Form and Job Sampling Agreement Form (if appropriate)
