Mad River Youth Soccer League Attention Soccer Players & Parents Registration for the Fall 2016 Season is under way! MRYSL serves children ages 4-17 (age groups determined by year of birth))
3 WAYS TO REGISTER 1) Print out 1601 registration form and mail it to us with payment or 2) Stop by our office or attend a registration event in May or 3) Register online starting May 1 at caln-madriver.affinitysoccer.com Registration events: Arcata: Thursday, May 26, 5:00pm – 7:00pm, Arcata Community Center Lobby McKinleyville: Saturday, May 28, 10:00 am – 12:00 pm, Ray’s Food Place, 1500 Anna Sparks Way MRYSL Office: Tuesdays and Thursdays 8:00 am to 1:00 pm 1309 11th St, Suite 202, Arcata (707) 822-3333 (do not mail to our physical address!) If registering in person or by mail, complete registration includes: Two copies or duplicates of signed Registration Form 1601 Verification of birth date (players who are new to MRYSL must provide copy of official birth certificate, Medi-Cal card, or passport) Check for registration made payable to MRYSL or Paypal receipt if paid on MRYSL.net Send to: Mad River Youth Soccer League P.O. Box 103 Arcata, CA 95518 Registration must be postmarked by May 31, 2016 Fees for 2016: First Child: $105.00
Second Child: $85.00
Each Additional Child: $80.00
Late Registration: After May 31, 2016 add $25.00. After May 31st, players will be placed on a waiting list and notified of team placement depending on availability and late processing time. Fees for unprocessed registrations will be returned by September 30, 2016. Financial Assistance: To be eligible your registration must be postmarked or received no later than May 31, 2016. Financial assistance forms are available at the registration sites or on the web.
