The following paragraphs will explain our enrollment process. We are confident that your experience with the Madison Parish School District will be a pleasant one. Please complete and sign the attached registration forms. This registration packet includes a Student
Enrollment Sheet, State-Required Demographic Information Sheel and Request for Records Fax Sheet. The applications for Free and Reduced Meals will be available at a later date. Due to the large number ofenrollments that we receive on a daily basis, we are asking that you please retum this packet to tle District Central OIIice, along with a COPY of the following documents to expedite your child's / children's enrollment.
