I have read and agree to the Acknowledgement of Responsibility Statement provided on Page 2 of this Application, as well as the La Mirada Youth Sports Code of Conduct. I understand that violations of my responsibilities may result in disciplinary action, as well as action by the full Board of Directors, including, but not limited to, suspension and/or expulsion from LMBA as specified in the rules and By-Laws of the League and/or PONY Baseball, Inc.

La Mirada Baseball Association Manager Application (Page 2) Acknowledgement of Responsibility I understand and agree that, if approved and selected to manager or coach a team for the La Mirada Baseball Association (LMBA), I serve as a representative of LMBA and PONY Baseball. I am aware of LMBA’s, PONY Baseball, Inc.’s, and the City of La Mirada’s principles and Codes of Conduct as they relate to managers, coaches, and other participants in LMBA – and I agree to uphold and abide by said principles and Codes of Conduct. I understand that, if selected as a manager, I will be working with youths and am responsible for setting an example of good sportsmanship, fairness, respect, and self-control – and, as such, restraints on my actions may be necessary in order to adhere to these principles. I understand the using crude, abusive or obscene language or gestures toward or in front of players, umpires, managers, coaches and/or spectators is strictly forbidden. I understand that corporal punishment is illegal and that LMBA is obligated by law to notify local authorities if it is used. I understand that any disciplinary action I intend to impose upon a player must first be approved by the Commissioner of my Division and/or the LMBA Board of Directors, depending on the severity of the problem. I understand that the use of tobacco products, alcohol, and/or drugs is prohibited at Los Coyotes. I may be called upon to assist in removing spectators engaged in the consumption of or under the influence of these products. If there is evidence that I have used these products before a game or practice, I may be asked to leave the field or be subjected to further disciplinary action including, but not limited to, forfeiture of my team’s game. I also understand that if I am approved as a manager, I may appoint a Head Coach, Assistant Coach(es) and Team Parent to assist me. I agree to inform the adults assisting me of the rules of LMBA, PONY Baseball, The City of La Mirada’s Youth Sports Code of Conduct and the Usage Agreement between LMBA and the City of La Mirada. I will also ensure that each member of my coaching staff, the Team Parent, and any other adult who has contact with the players (whether during the game or during any practices) are fingerprinted and cleared prior to working with the children. I further understand that I am responsible for the actions of my assistants and spectators attending the games and practices – and that I may be requested to assist in removing such persons violating any of the applicable rules, regulations, or Code of Conduct. I am aware that all games will be played in accordance with the most recently adopted edition of “LMBA Official Rules and Regulations”, “Pony Baseball Rules & Regulations”, and MLB rules – and in that order. I will do my best to act responsibly and according to those rules. I understand that the umpire is in charge of the field and that his/her ruling may not be excessively challenged during the game. If I feel that an umpire has misinterpreted the rules, I can protest the game and have the ruling reviewed by the LMBA Rules Committee as outlined by the by-laws of LMBA. I understand that excessive challenges are grounds for the umpire to eject me from the game. I understand that transportation of children other than my own, to / from games and/or practices is strongly discouraged. I will discourage this of other parents on my team as well. This is for the protection of the children and protects the adults from any accusation of wrongdoing. I understand that my failure to adhere to applicable rules, regulations, principles, and Code(s) of Conduct could result in disciplinary action against me – including suspension and/or expulsion from LMBA.
