I authorize The Skin Institute & Day Spa doing business as Neroli Salon & Spa, to charge the credit card above based upon the service provider pricing Membership Fees on the first of each month for the duration of my trial membership, and I hereby acknowledge and agree to the Trial Membership Terms and Conditions set forth below. I agree to promptly provide to Neroli Salon & Spa in writing any change in credit card or account information. I understand that a delay or failure in payment will result in suspension or termination of my membership and all membership benefits. __________ Initial Here TRIAL MEMBERSHIP TERMS AND CONDITIONS PLEASE READ THE FOLLOWING TRIAL MEMBERSHIP TERMS AND CONDITIONS (the “Terms and Conditions”) RELATING TO YOUR TRIAL MEMBERSHIP CAREFULLY. By authorizing The Skin Institute & Day Spa doing business as Neroli Salon & Spa(“Neroli”, “us” or “we”) to charge your credit card, as indicated above, you agree to the Terms and Conditions set forth herein. We reserve the right, in our sole discretion, to change, modify, add or remove provisions of these Trial Membership Terms and Conditions or the terms of our Neroli Membership program at any time. In the event of such change to these provisions or our Membership Program, we will notify you. If you do not agree to these Terms and Conditions, or any subsequent modification of these Terms and Conditions or our Membership Program, as applicable, you should not authorize us to charge your credit card, or you should arrange to cancel your membership with us, as applicable. The term of your trial membership is three months. Members will have an option to extend their three month membership to an annual membership upon conclusion. Thereafter membership can be renewed annually based on terms and conditions provided by us. Subject to the cancellation fee described below, you may cancel your trial membership at any time. Cancellations shall be effective for any month upon written notice to us received within 14 days prior to the first day of such month in which cancellation will be effective. All membership cancellations shall be made in writing and submitted to the Neroli Salon & Spa, 327 East St. Paul Avenue, Milwaukee, WI 53202. Cancellations made before the expiration of the then-current term will be subject to a cancellation penalty equal to three months of the Membership Fees. A valid driver’s license and credit card are required to enroll. All members must be 18 or older. In addition to the Membership Fees, you agree that you will be responsible for payment at the time of service for any additional services, add-on services or upgrades provided to you in connection with the membership benefits and services. Monthly Membership Fees shall be automatically charged to your credit card or other account on the first of each month during the term of your membership, and any renewal term, as applicable. Cancellations with less than 24 hours notice for any reservation for services included with your membership are subject to penalty of forfeiture of all remaining membership benefits and services for that month. Monthly membership services and benefits shall not accrue and must be used within the designated calendar period to which such services and benefits are applicable. Failure to use any service or benefit during the designated time period for which it is applicable shall be deemed a forfeiture of such service or benefit. We reserve the right to revoke, terminate or suspend any membership, including, without limitation, due to any declined payment. Membership benefits cannot be combined with any additional promotional offers. All applicable salon rules, regulations and policies in effect from time to time shall apply to the services. Additional restrictions may apply. None of your membership rights, benefits or services may be assigned to any other person, whether by operation of law or otherwise, without our prior written approval, which may be withheld for any reason. If any term of these Terms and Conditions shall be judicially determined to be illegal, invalid or unenforceable at law or in equity, it shall be deemed to be void and of no force and effect to the extent necessary to bring such term within the provisions of any such applicable law or laws, and such terms as so modified and the balance of the terms of these Terms and Conditions shall remain enforceable.
