Important Information about Grandfathered Humana Individual Major Medical Discontinuances As an agency with agents who sell Humana IMM plans, we want you and our members to be prepared for some upcoming product changes regarding certain Grandfathered Individual Major Medical plans that were sold with effective dates prior to March 23, 2010. Some Grandfathered plans will no longer be available starting in 2016. Members whose plans will end will receive a letter from Humana no later than 90 days prior to the plan termination date. Prior to December 01, 2015, Humana will be sending letters to members whose plans will end on February 29, 2016. Members will have the option to choose a new policy through you, their Humana agent/agency or explore other options through the Federal Health Insurance Marketplace. The 2016 Open Enrollment Period (OEP) is from November 1, 2015 through January 31, 2016 and these members can enroll in a new policy during this time. However, if they do not enroll during the OEP they will also be eligible for a Special Enrollment Period (from 60 days prior to their plan termination date to 60 days after their plan termination date). To help ensure there is not a gap in coverage, members will need to apply for a new plan by the 15th of the month prior to their termination date noted in their letter. It is important for you, their agent, to help protect clients from going without coverage and incurring tax penalties and to preserve your book of business. Summarized below are the main points that you and you agents need to know so that this important information can be shared with clients about their Grandfathered Individual Major Medical plan. • Grandfathered: All Individual Major Medical Plans issued with effective dates prior to March 23, 2010 in Colorado will terminate February 29, 2016. • Grandfathered: All Individual Major Medical Plans issued with effective dates prior to March 23, 2010 in AL, AZ, FL, MS, OH, OK, SC, TN, WI will begin terminating as of the March 2016 renewal and will continue monthly through 2017. • Grandfathered: All Individual Plans sold in Georgia between 8/25/2007-3/23/2010 in the following counties (Banks, Barrow, Bartow, Bibb, Bryan, Bulloch, Burke, Butts, Carroll, Chatham, Cherokee, Clarke, Clayton, Cobb, Columbia, Coweta, Crawford, Dawson, DeKalb, Douglas, Effingham, Fayette, Floyd, Forsyth, Fulton, Glynn, Grady, Greene, Gwinnett, Habersham, Hall, Hancock, Harris, Heard, Henry, Jackson, Jenkins, Jones, Lamar, Laurens,

Madison, McDuffie, Monroe, Morgan, Muscogee, Newton, Oconee, Paulding, Peach, Pickens, Polk, Richmond, Rockdale, Screven, Spalding, Thomas, Union, and Walton) will begin terminating as of the March 2016 renewal and will continue monthly through 2017.

What are the Member's Options? Enroll in a new plan during the 2016 OEP (11/1/2015-1/31/2016) or through their Special Enrollment Period by: • Choosing a new On-Marketplace policy through you, their Humana Agent or exploring other plan options through the Federal Health Insurance Marketplace by visiting Healthcare.gov. • Choosing a new Individual Medical policy outside the Marketplace through you, their Humana Agent or by exploring other plan options available to them.
