Message from the President The Foundation celebrated its 20th anniversary during 2012, and continues helping young tennis players improve their lives through tennis and educational opportunities. I want to thank our volunteers, members and tennis enthusiasts who gave of their time and/or financially supported the Foundation this year. Through the generosity of our donors we were able to disburse a record $120,000 in grant funding. Approximately $77,000 in program funds were awarded in 2012, supporting 35 organizations and over 5,000 young people. We funded 32 junior players in the amount of nearly $44,000. This year’s Annual Report contains several examples regarding how funded programs used their grant money to start or enhance tennis opportunities for youth in their local areas. Also, several current and former Midwest juniors who received funding through our Foundation are featured in this Annual Report regarding their success on the tennis court this past year. In addition to providing individual and program grants, the Foundation annually awards scholarships to high school seniors to advance their educational and tennis interests. The Foundation also provides funding to several Midwest high performance players to compete on USTA/Midwest Section teams in three national championship events against other junior teams from around the country. Also, the Foundation funds select junior camps led by various Midwest coaches. In an effort to expand our impact and funding opportunities, the Foundation has several fundraising initiatives. First, a Charity Gala was created five years ago which includes a silent auction, live auction, and program/ad book. The five Charity Gala events have raised approximately $300,000 towards our mission. Second, an Annual Campaign provides another source of funding for grassroots tennis programs and junior players. The Annual Campaign continues to build a solid foundation of donors for the future. Third, leveraging the US Open continues to be another avenue for the Foundation to generate financial support through the lease of a suite at Arthur Ashe Stadium, and individual donation ticket packages. Thank you for assisting the Foundation in its mission, and your dedication and support of the great sport of tennis. Sincerely,

Steve Butzlaff President 2

Grants and Programs The Foundation provides funding to youth tennis programs and individuals to:  Serve areas not presently serviced by other or similar tennis organizations;  Increase opportunities for junior tennis players;  Support recreational junior tennis players;  Fill a gap in the developmental continuum of young tennis players; and  Assist junior tennis players in need of financial support to reach their highest potential.

MYTEF Grants Assist Programs Offering Tennis to Youth The Foundation is dedicated to supporting youth tennis programs and activities throughout the USTA/Midwest Section. The following 2012 program recipients share how the Foundation assisted them this year: “Our program received a $1,000 grant, and the Foundation was one of the first organizations to grant funding to our program. This support allowed us to provide partial scholarships to our 19 participants who otherwise could not have afforded the program. Our summer program was held at a local facility where we set up QuickStart tennis courts outside in the parking lot (reinforced tennis may be taught anywhere). Children 10-and-under were introduced to tennis using the QuickStart Tennis format.” – Grandparents United Together Strongly in Detroit, Mich. “The $5,000 grant we received was crucial to our program’s success as we had a record 430 youth participate this year. This record number of youth was a result of increased advertising of our program through flyers and information in the local schools, newspaper, police website, and city utility bill mailings. We used the Foundation grant money for instructor costs, supplies including program t-shirts, and a special tennis event. Our summer program is offered five days a week, and includes lunch as well as other physical exercise. Approximately ten students from the program were awarded sponsorships from our organization to attend the USTA/Midwest Section Junior Excellence Camp and Player Development Camp.” – South Bend Police Athletic League in South Bend, Ind. “Our CTA was awarded a $3,000 grant to help offset costs associated with our Midwest Youth Team Tennis and Futures Team Tennis programs. We used the funding for instructor fees, equipment including racquets and balls, t-shirts, supplies, and medals. The Foundation grant allowed us to use the money raised locally in our community to provide scholarships to children who otherwise couldn’t have afforded the program cost. There were 140 children who participated in our program which utilized the QuickStart format for all 10-and-under youth. Many parents commented the medals given to all participants made the kids feel like winners on the court, and their children thought tennis was so much fun.” – Auglaize Area Tennis Association in Wapakoneta, Ohio “The $2,000 grant we were awarded enabled us to purchase all the equipment needed for our new 10-and-under program which used the QuickStart format. Our summer program introduced more than 200 inner city youth to the sport of tennis. All youth participated in nutritional and healthy living workshops, and were provided a healthy lunch two times per week. We marketed our program through flyer distribution within the community and advertisements in the local newspaper. Our goal is to expand our program to include junior high and high school students.” – Louis Valentine Boys’ and Girls’ Club of Chicago in Chicago, Ill.

“We appreciate the $3,000 grant we received which helped purchase equipment, pay instructors, and buy necessary supplies. Our program operates year round, which includes using the school gym during the school year, and outdoor public courts in the summer. Approximately 100 youth from Hispanic and other ethnicities enrolled in our summer program, and we implemented the QuickStart format for the first time. Our program helps build the fitness, self-confidence and responsibility of young kids.” – Camp Esmeralda in Elm Grove, Wis.

Foundation Provides Financial Assistance to Individual Players The Midwest Youth Tennis & Education Foundation has supported youth since 1994. Over the past 20 years, approximately $425,000 has been awarded to players of all ability levels. One of the ways the Foundation has supported youth has been awarding financial assistance to high-level players in need of funding to travel and compete in tournaments. Following are some current and past individual grant recipients achieving success on the tennis court this year. Brooke Austin of Indianapolis, Ind. – Austin is currently ranked No. 1 in USTA Girls’ 18 and USTA/Midwest Section Girls’ 18 singles. Austin captured the USTA National Spring Championships Girls’ 18 singles title, and was the Easter Bowl ITF Championship Girls’ 18 singles finalist. She won the USTA/Midwest Section Closed Girls’ 18 singles championship. She qualified for the 2012 US Open Junior tournament, and earned a wild card into the 2012 US Open women’s draw. She represents the USTA at national and international tournaments, and competed for the USTA/Midwest Section at the USTA Girls’ 18 National Championships. Evan King of Chicago, Ill. - King earned Intercollegiate Tennis Association (ITA) All-American honors for the third consecutive year. He was named the Big Ten Conference Athlete of the Year, and named to the All-Big Ten team for the third straight year. As a USTA/Midwest Section junior player, King won numerous national junior titles, captured the prestigious USTA/Midwest Section Closed Junior Championships, and finished No. 1 in each of the USTA/Midwest Section Boys’ 12, 14, 16 and 18 age divisions. Edward Covalschi of Macomb, Mich. – Covalschi is currently ranked No. 1 in USTA/Midwest Section Boys’ 18 singles. He won the USTA/Midwest Section Closed Boy’s 18 doubles championship. He competed in the USTA National Championships this past summer. Covalschi has represented the USTA/Midwest Section at several national team competitions. He signed a letter of intent this fall to play collegiate tennis at the University of Notre Dame in 2013. Nicole Gibbs of Shaker Heights, Ohio – Gibbs won the 2012 NCAA Championships singles and doubles titles becoming only the third player in NCAA history to capture both titles in the same season. She was named a 2012 AllAmerican in singles and doubles, Honda Sports Award recipient for women's tennis, ITA National Player of the Year, and Pac-12 Player of the Year. Gibbs competed in the 2012 US Open women’s main singles draw. She plays professional WTA and ITF events while maintaining her amateur status. Rajeev Ram of Carmel, Ind. – Ram advanced to the quarterfinal round of the 2012 Australian Open Men’s Doubles main draw. Ram has claimed six ATP doubles titles in his career, and his first title was won in 2009 in Newport, R.I. As a USTA/Midwest Section junior player, Ram won a total of ten national junior titles,

7

captured the prestigious USTA/Midwest Section Closed Junior Championships, and finished No. 1 in each of the Boys’ 12, 14, 16 and 18 age divisions. In addition to awarding financial assistance to high-level players, the Foundation also assists local players striving to improve their skills, play more tournaments, and enjoy participating in their favorite sport. Following are comments from two junior players who received grants this year to play in a local indoor program and compete in more tournaments. “I love playing tennis and want to play more, but my family cannot afford for me to play as much as I would like without assistance. There are not many tournaments available in my area, and competing will require me to travel outside of my state. Last year I sent a letter to family and friends asking them for support by sponsoring me or collecting aluminum cans for me which raised $300 towards my tennis expenses. This year in addition to collecting aluminum cans, I started making and selling tennis ornaments to raise money for my tennis expenses. I hope to someday become a tennis coach or instructor. I carry my old QuickStart balls with me when playing at the courts in my town, and give some to any kids I see playing on the courts for the first time.” 11-year-old Canaan from Indiana “A grant would help me afford lessons at the local indoor program in my area, and provide me the opportunity to compete in tournaments. I practice more than other kids my age, and often beg my dad to go to the park and play tennis with me after he gets home from work. Although I am young, I love tennis, and will do everything I can to keep learning and improving my skills. I was excited to be invited to participate in a tennis league at my school this fall. As I get older I want to do what other older players do which is teach little kids like me to have fun and improve my tennis game.” 9-year-old Allison from Illinois

Gullikson, Hooked on Tennis, and Van Horn Scholarships The Tim and Tom Gullikson Scholarship was established by the Midwest Youth Tennis & Education Foundation in January 1998 to honor Tim and Tom Gullikson, and to reward young adults with funding to advance their educational and tennis interests. Each year, scholarships are presented to two tennis players from the USTA/Midwest Section who meet demanding academic and tennis achievement criteria. This year Monica Chan of South Bend, Ind., and Luke Hubert of New Berlin, Wis., were awarded the Tim and Tom Gullikson Scholarships.

The Hooked on Tennis Scholarships were established in October 2010 to support two junior players with funding to advance their educational and tennis interests in college. This was the inaugural year for the presentation of these scholarships.

8

Andrea Fetters of West Milton, Ohio, and Michael Schnarr, Jr. of Madison, Wis., were the inaugural recipients. The Rick Van Horn Scholarship was established in 2008 to support one junior player with funding to advance his/her educational and tennis interests in college. Shelby Fenn of Bowling Green, Ohio, was this year’s scholarship recipient.

Spring Camp

The USTA/Midwest Section Spring 12-and-under Camp was held in Boca Raton, Fla., from April 5-7. This camp provided training opportunities to those players participating in the 2012 USTA Spring National Championships held April 8-13. This year the camp had 19 participants. The Midwest coaches included Stephanie Hazlett from Evansville, Ind., and USTA/Midwest Section Director of Junior Tennis Chad Docktor.

Team Support Each year the USTA holds three National Junior Championships where high performance teams from each of the 17 Sections compete against each other. Players who are selected to represent the USTA/Midwest Section are provided funding to offset the costs of their participation. The following teams currently receive financial support from the Foundation:  USTA Boys’ and Girls’ 16 Intersectionals — four boys and four girls competed in Shreveport, Louisiana;  USTA Boys’ 18 National Team Championships — six boys competed in Champaign, Illinois;

9

 USTA Girls’ 18 National Team Championships — six girls competed in San Rafael, California. Our combined Intersectional team captured the title at the 2012 competition. The Boys’ 18 National Team Championships squad brought home second place this year, and Girls’ 18 National Team Championships members finished in third place.

Junior Excellence Camp The Junior Excellence Camp was held from September 28-30 at the University of Notre Dame in South Bend, Ind. The event brought together 68 of the most talented and diverse junior players within the USTA/Midwest Section. Lead coach Ryan Sachire, University of Notre Dame Associate Head Men’s Tennis Coach from South Bend, Ind., provided instruction similar to what is used by coaches at USTA High Performance Camps during Saturday’s two training sessions. On Sunday morning, a round robin team tennis competition was held, and players implemented the skills they learned during the camp. Thank you to Gamma for being a sponsor of this year’s camp. Gamma provided string and grips to all participants, and allowed the junior players to demo racquets. All participants and coaches also received a tour of the University of Notre Dame during the weekend.

USTA Serves USTA Serves is the charitable arm of the USTA. Each year USTA Serves awards college scholarships to high school seniors and incentive awards to youth in grades 6-11. In 2012, several USTA/Midwest Section juniors were awarded scholarships or incentive awards: Marian Wood Baird Scholarship – Kaitlyn Devine of Bethalto, Ill. ($15,000) College Education Scholarship – Arielle Williams of Southfield, Mich.; Emily Hansen, Oak Creek, Wis.; and Nicholas Wiand, Elwood, Ind. ($6,000) College Textbook Scholarship – William Schultz of Waukesha, Wis. ($1,000)

MY Gift to Kids’ Tennis Initiative The Foundation launched a new fundraising initiative from October 15 through December 1 aimed at raising awareness and interest in local District programs, and highlighting how the Foundation can be a contributing partner in the communities it services throughout the Section. Each of the USTA/Midwest Section Districts were provided an opportunity to highlight one worthwhile program. The Foundation website page included a list of all the dynamic junior programs in the spotlight during this fundraising opportunity. Prospective donors were provided the opportunity to make their own online financial contribution to the District program in their area and help grow junior tennis in their District. Thank you to everyone for supporting this initiative, and we are excited to announce donations raised by the deadline were $7,031. Also, a special congratulations to the People for Palmer Park in Detroit (Southeastern Michigan) as their Palmer Park Tennis Academy program generated the most donations, and this program received an additional $1,000 from the Foundation.

Fundraising Activities In an effort to expand our impact and funding opportunities, the Foundation has several fundraising initiatives. First, the MYTEF Charity Gala was created five years ago. The Annual Charity Gala includes a program/ad book, silent auction, and live auction. The five MYTEF Charity Gala events raised approximately $300,000 towards our mission. A committee oversees the charity event which is held each December. The 2012 Charity Gala Committee did a great job ensuring the gala’s success despite the challenging economic environment. This year’s gala included an online silent auction held prior to the actual gala event. The online silent auction was then transitioned to an on-site silent auction. Second, the Foundation continued the Annual Campaign to build upon relationships with current donors, and start developing relationship with new donors. This campaign provides funding for grassroots tennis programs and junior

11

players. Letters and pledge forms for the Annual Campaign were mailed in December to all Midwest donors from 2010 and 2011, past grant and scholarship recipients, current USTA/Midwest Section organization members, and the USTA/Midwest Section volunteer/staff family. The Annual Campaign continues to build a solid foundation of donors for the future. The 2012 Annual Campaign has netted $6,500 through the end of the year. Third, several US Open packages were again provided by our volunteers (which include invitations to the President’s Suite) to be sold to prospective donors. Proceeds from the sale of US Open packages provided by our volunteers the past few years have been earmarked towards the Charity Gala events, and played a key role in the first two Charity Galas generating $150,000 toward our mission. Fourth, the Foundation also leases a suite at the US Open in Arthur Ashe Stadium. It annually subleases the suite, using the proceeds to provide funds to young players and programs. This year the net contribution from the suite lease was $75,000. Several individuals also donate their unused US Open tickets, and the Foundation sells them to organizations and individuals in need of tickets. In addition to the preceding fundraising strategies, the rental income associated with long-term leases of our tenants secured in the Foundation building provides a continuing revenue source for the Foundation. This year the Foundation continued to pay down the mortgage so additional grants and scholarships can be awarded to young people. In the next two years, the Foundation will seek to further expand our fundraising strategy to provide the broadest possible range of opportunities for donors to support us.
