Dear Parents/Carers Your daughter has an upcoming excursion as a part of the 2016 Outdoor Education Classes. The Mount Carmel Outdoor Education program aims to provide practical and active learning experiences in natural environments. The program facilitates the development of skills and understandings to participate safely and respectfully, with an appreciation and awareness of natural environments. The development of self-confidence, teamwork and leadership skills are key objectives underpinning each student’s participation in this Unit of Study. The next four excursions in this program will involve a practical experience in Indoor Rock Climbing at Rock It Climbing Centre. Experienced and qualified instructors will be used in the activities during the excursions. Attached is a Wavier form from Rock It Climbing Centre, both permission form and wavier. TRANSPORT ARRANGEMENTS:  Transport to and from the location will be via the college bus. HEALTH / MEDICAL INFORMATION:  Please advise the College Office or myself of any injury, illness or conditions that may impact on the way in which your daughter can participate in Extreme Sports lessons.  If your daughter has any medication, such as asthma puffers, could you please ensure she brings this to each session. Please complete and return the Rock It Climbing Centre Wavier form & Permission slip below prior to the first activity. I will ensure that the College has up to date information regarding my daughter’s medical status and medication requirements. Dear Parents/Carers Rock It Climbing Centre - Liability Release Form Please be advised that when you sign the attached Liability Release Form from Rock It Climbing Centre, you are agreeing to the terms contained within the form. The intention of the form is to release Rock It Climbing Centre from liability in the event that their negligence contributes to an injury to any participant. When you sign the form you are acknowledging that you will not be entitled to access Rock It Climbing Centre’s insurance cover. As Mount Carmel is not a signatory to the Liability Release Form, the College’s insurers will also not be able to provide cover. Please note that this Liability Release Form is a standard and widely used form. In the event that Rock It Climbing Centre is liable for an event occurring which results in injury, it is recommended that you challenge the waiver to pursue compensation against them. As your daughter is participating in Rcok Climbing at Rock It Climbing Centre, as part of the 2016 Outdoor Education Classes, it would be appreciated if you would acknowledge receipt of this advice by signing below, complete the form on the reverse and return it to the College Office by 30 May 2016. Please note, your daughter will not be able to participate in this event if the Liability Release Form is not forwarded to the College prior to the excursion.
