1. Make a copy of this form for your records. 2. Tickets are $14 each. 3. Please put this form and money in manila envelope (provided to Team’s Manager with Team Name and Division printed on it). It will be returned to you with your tickets. 4. Drop off Ticket Form and Money envelope at Crittenden Fields on Friday, April 7th from 5:30 – 7:30, or Saturday, April 8th from 10:00 am – Noon. Please use manila envelope that contains TEAM NAME and DIVISION. 5. Ticket pick up will be at Crittenden on Friday, April 28th from 5:30 PM – 7:30 PM (Rain or Shine).
Ticket Forms and Money MUST be in by April 8th! Unless other arrangements have been made with the Nat’s day Coordinator, No Orders can be processed after this date.
