Failure to volunteer may result in my child not participating There may be additional cost involved in Regional or National Games If team advances to Regionals, I must be available the week of Thanksgiving, including Thanksgiving Day If team advances to Nationals, I am aware games may involve missing school I must use the 2017 Pop Warner Medical / Physical Form I must participate in the North Manatee Fundraiser I will adhere to the North Manatee Storm Code of Conduct at all events that involve the North Manatee Storm

Your child will not be allowed to participate or receive equipment until all paperwork and payment is turned in to the registrar.
CODE OF CONDUCT FOR PARENTS AND SPECTATORS
I will treat all volunteers, including coaches with respect. Any issues I have will be raised quietly away from children.
I will treat all parents with respect. I will inform a board member if I have any issues with other parents.
I understand that my children are playing a game. I will root for my children and their team, and will refrain from disparaging any other players.
I am the most important role model for my children. Above all I will demonstrate good sportsmanship and a strong work ethic that my children may look up to.
If I fail to adhere to the North Manatee Storm Code of Conduct, I will voluntarily leave the facility. I understand that this is in the best interest of my child and their teammates.
