CONDITIONS OF FINANCIAL AID: I understand that to qualify for this financial aid at Nova Southeastern University, I must:

Fulfill the admission requirements for the Undergraduate or Graduate Degree Programs. Meet and maintain the eligibility requirements for athletics participation and financial aid established by the NCAA, Sunshine State Conference or Peach Belt Conference and Nova Southeastern University. 3. Complete the Free Application for Federal Student Aid (FAFSA) and all forms of financial aid, and complete the financial aid process. If the student-athlete receives any other financial assistance, this athletic award is subject to revision based on the need factor as established by the Office of Financial Aid and the maximum allowance of aid as set forth by the NCAA. 4. International students who receive scholarships must be aware of potential Non-Resident Alien (NRA) withholding tax. All colleges and universities in the United States are required by the Internal Revenue Service (IRS) to withhold 14% of any scholarships issued for the purpose of non-qualified expenses. Qualified expenses are tuition, fees and books. Exceptions to this rule will be based on Income Tax Treaty country status. There are required forms regarding Tax Treaty status and US Tax Returns that an International Student must fill out. International students should consult with the NSU Office of International Students and Scholars to determine their status and required forms. The required 14% withholding tax will be due to NSU at the time that nonqualified aid is disbursed to the student’s account (international students only). 5. Period of institutional financial aid. The financial aid contained in this letter is awarded for a period of one academic year only (NCAA Bylaw 15.6.3.1). The Department of Athletics may recommend to the financial aid authority that financial aid be renewed each year for a period of four years, however, you are hereby informed that the renewal will not be automatic (per NCAA Bylaw 15.6.3.1.1) and will remain at the discretion of t he Department of Athletics. 6. Reduction or Cancellation permitted. The Department of Athletics reserves the right to reduce or cancel institutional financial aid during the period of the award, per NCAA Bylaw 15.6.4.1., for reasons of misconduct, misrepresentation, ineligibility or voluntary withdrawal by the s tudent-athlete. 7. The student-athlete shall be subject to all academic rules and requirements for eligibility regarding registering credits and attained credits. Academic standing, grades, earned hours and grade point average resulting in ineligibility could be cause for cancellation of award. 8. The student-athlete shall abide by all team rules and regulations set forth by the coach, adhere to athletic policies/procedures set forth by the Department, and must be eligible to compete by NCAA, SSC/PBC and NSU rules for this aid agreement to be binding 9. The student-athlete shall represent NSU in a respectable manner at all times. Failure to do so can result in dismissal from the team and loss of athletic aid. 10. This athletic award letter constitutes a complete and final agreement pertaining to the athletic scholarship award for the current academic year as indicated. This document supersedes any prior written and/or oral agreement(s) pertaining to the student’s athletic scholarship offer
