TEAM 3 NOTES FROM PROGRESS ENERGY CAPE FEAR PLANT INSPECTIONS ON 12/15/09
1985 dike US slope - minor wave erosion in some locations. Dam crest - no problems. DS slope - Several areas of sparse vegetation and erosion . The erosion appeared to be potentially due to "jugs" formed by the presence of dispersive clays on the lower two-thirds of the dam face. The engineer (AI Tice of MacTec) reported that he believed som e additional soil was wasted along the lower two-thirds of that portion of the dam. A view of the profile of the DS slope surface appeared to show a slight flattening of th e slope near the toe which would tend to support that hypotheSis. Also observed were w hat appeared to be small rodent burrows and some areas of shallow sloughing. Toe contact - Observed areas of possible seepage that should be monitored. Wet conditions made it difficult to determine characterize the seepage, but no active flow was observed. It is recommended that it be monitored in t he future. Also noted was som e mate rial that appeared to have settled from the "jugs" that eroded in the areas on the DS slope. Another area of potentia l seepage was observed just to the left (fa ci ng downstream) of the vehicle t urn-out where we parked. This is very near to the PS barrel an d should be monitored in future inspections as well to insure it is not a developing piping failure. No other issues noted except for the absence of a bottom drain . 1956 ash pond dike General notes: Dam is no longer impounding and nearly full of old coal ash. Unclear if it is capable of im pounding any significant volume of water. No observed water impounding even after recent rains. Trees of all sizes on DS slope, toe, and in the impoundment area itself. DS slope ave rages about 1.5H:1 V, but is much steeper in place s. Animal slides and sloughs observed at multiple locations. Some apparent migration of trees down the slope is evident {"bent trees"}. A few large downed trees with portions of OS embankment damaged due t o root ball. Significant erosion obse rved at several locations.

TEAM 3 NOTES FROM PROGRESS ENERGY CAPE FEAR PLANT INSPECTIONS ON 12/15/09
1985 dike US slope - minor wave erosion in some locations. Dam crest - no problems. DS slope - Several areas of sparse vegetation and erosion . The erosion appeared to be potentially due to "jugs" formed by the presence of dispersive clays on the lower two-thirds of the dam face. The engineer (AI Tice of MacTec) reported that he believed som e additional soil was wasted along the lower two-thirds of that portion of the dam. A view of the profile of the DS slope surface appeared to show a slight flattening of th e slope near the toe which would tend to support that hypotheSis. Also observed were w hat appeared to be small rodent burrows and some areas of shallow sloughing. Toe contact - Observed areas of possible seepage that should be monitored. Wet conditions made it difficult to determine characterize the seepage, but no active flow was observed. It is recommended that it be monitored in t he future. Also noted was som e mate rial that appeared to have settled from the "jugs" that eroded in the areas on the DS slope. Another area of potentia l seepage was observed just to the left (fa ci ng downstream) of the vehicle t urn-out where we parked. This is very near to the PS barrel an d should be monitored in future inspections as well to insure it is not a developing piping failure. No other issues noted except for the absence of a bottom drain . 1956 ash pond dike General notes: Dam is no longer impounding and nearly full of old coal ash. Unclear if it is capable of im pounding any significant volume of water. No observed water impounding even after recent rains. Trees of all sizes on DS slope, toe, and in the impoundment area itself. DS slope ave rages about 1.5H:1 V, but is much steeper in place s. Animal slides and sloughs observed at multiple locations. Some apparent migration of trees down the slope is evident {"bent trees"}. A few large downed trees with portions of OS embankment damaged due t o root ball. Significant erosion obse rved at several locations.
