Online Rebate Submission Instructions 1. Please go to www.BrandSourceRewards.com to submit your claim online. Remember to provide a valid email address which will be used to inform you of updates to your claim status. Claim MUST be submitted no later than March 26, 2018 based on purchase date. No adjustments for claims submitted on time will be accepted after April 26, 2018. There are no extensions for claims submitted after 30 days of purchase date. 2. Eligible models must be purchased within the dates of this promotion (See program details above). 3. Include a legible copy of your sales receipt showing all charges paid, including model numbers, serial numbers, dealer name, and purchase date. You will also need to enter the AVB ID (listed on this form by the selling dealer). Missing or incomplete information voids rebate offer. 4.
