Held in conjunction with the USTA Hawaii Pacific Section’s Tennis Weekend, this round-robin team competition will benefit the Susan G. Komen for the Cure ®, the world’s largest grassroots network of breast cancer survivors and activists.

EVENT FORMAT & REGULATIONS • Teams of four players or more (minimum 2 Women & 2 Men) • A (NTRP rating 4.0 & Above) & B flights (3.5 NTRP rating & lower) • Round Robin format – 3 games per match • Matches are Women’s Doubles & Men’s Doubles • Tiebreak matches to be decided by Mixed Doubles • $100 per team entry fee • Entry deadline EXTENDED to Saturday October 4, 2008 • For more information visit www.hawaii.usta.com or call (808) 955-6696

Waiver of Responsibility Each applicant must sign. Acceptance of my entry in these events is without assumption of responsibility of any kind by the USTA, the USTA/Hawaii Pacific Section, the host club owners, trustees or governing boards of facilities, committees, or the management of any event in which I may be entered or may participate. In consideration of the acceptance of my entry, I do hereby for and on behalf of myself and my heirs and legal representatives release and forever discharge the USTA, USTA/Hawaii Pacific Section, and host from any and all claims and demands of every kind, nature and character excepting gross negligence, reckless conduct, or willful, wanton conduct, which I may have or may hereafter acquire for any and all damages, losses or injuries which may be suffered or sustained by me in connection with my activities during the period for which such permission is granted and any period traveling to or from such event described, and all such claims are hereby waived and released and I covenant not to sue therefor. I understand that this tournament will be governed by applicable USTA rules and regulations, the rules and regulations of this tournament, the rules and procedures governing discipline of players in USTA/Hawaii Pacific Section sanctioned tournaments, the USTA Code of Conduct, tournament policy, and ranking regulations, and agreed to conduct myself accordingly.
