Problem Resolution Form This document will be used to notify the Hartland Jr. Football Head Commissioner of any problem that has not been resolved to the satisfaction of those involved. This form will remain confidential. The information contained on the form will be forwarded down to the Color Commissioner/ Color Coordinator to which you are assigned and handled at the lowest level possible before accelerating to the Hartland Jr. Football Executive Board. This form is used to document the problem, and ensure a resolution. All forms will be kept in a confidential file until all parties involved have transited out of the Hartland Jr. Football program. Instructions: Please fill out the document completely listing dates if applicable. Once complete, place in a sealed envelope, and mail to HAYAA Jr. Football or scan and Email to: [email protected]
