Each parent will be responsible for going to the online store and placing an individual uniform order and paying for the uniform themselves. The coach will set the campain dates (start and end of order) and will also be responsible for making sure no two players pick the same jersey number.
INSTRUCTIONS: This form is used to set up your team store with BSN Sports. If you need replacement uniforms from last season you can contact Frisco Sports Center. Once BSN receives this form and your online store is set up, you will be sent an email with instructions to send to the players on your team to order their unifom. Parents will have to order prior to the end of the campain date. 1. Complete form. If you have questions call Robert Malvaez at 972-406-7115. 2. Scan and email form to [email protected] and copy [email protected]. 3. A Star Package is included with the t-shirt order and does not need to be ordered separately. 4. The Campain Date is the date you want the store to open and close. The sooner it closes, the sooner you will get your uniforms. It is recommended that you communicate this date clearly to your team and only leave the store open for one week. Once the store closes it will take 2-3 weeks to receive the uniforms. Uniforms will be shipped to the address listed above. 5. ORDER DEADLINE: NOV 6 Any order that closes after November 6th may not arrive prior to the first game.
