Registration Form Thank you for choosing Pleasant City Church MMO! Our hours are from 9:00am to 1:00pm, Monday through Thursday. Our ages include 6 months – Pre-Kindergarten. Please fill out the following registration form to enroll your child (one per child). A non-refundable $50.00 registration fee is required at time of enrollment to secure your child’s spot. We offer family discounts for siblings who are enrolled at the same time (discounts are listed below). For payments, please use the tuition envelope that is provided to you.
