Quartz Hill Shootout T-Shirt Order Form NOTE: T-shirts will be provided for the players and coaches upon completion and timely return of this form. Deadline for ALL T-shirt forms is April 3rd, 2018. If this form is not received by April 3rd, 2018, T-shirts will be furnished for your team but the sizes will be based upon other players and coaches in the same age bracket. Extra T-shirts may be ordered, but payment must accompany this form. A limited number of T-shirts will also be on sale at the tournament. You will receive a maximum of 10+2 T-Shirts for U10 teams, 12+2 T-Shirts for U12 teams, 15+2 T-Shirts for U14 teams, and 18+2 T-shirts for U16-U19 teams. SECTION:
