I, ________________________________________________ (parent/guardian of minor participant), herby agree to the following terms in relation to checkout of lacrosse equipment from Red Wing Area Lacrosse Association (RWALA): 1. I agree to examine the equipment and to notify RWALA before use if there is any reason to doubt its serviceability or safety. If at any time my child feels something doesn’t fit right, I will return the equipment and trade it for better fitting equipment. 2. I agree to assume all responsibility for the equipment while out of the possession of RWALA and promise to return to RWALA such equipment in as good condition as it was at the date of the check-out, natural wear to be expected. 3. I shall be responsible for any loss, theft, damage or destruction of the loaned equipment and shall not loan, sublet or in any manner dispose of the loaned equipment. I understand that if the equipment is damaged beyond normal wear, is lost, or is stolen, I will be responsible for the full replacement cost of the equipment, per the table on page one (1) of this form, and that my security deposit may be forfeited as part of that payment of full replacement cost. 4. I agree to return or recheck the loaned equipment to RWALA in a timely manner at the end of the current season. If I fail to return or recheck the equipment within one (1) week of the end-of-season check-in deadline noted below, I understand I may be responsible for the full replacement cost of the equipment, per the table on page one (1) of this form, and that my security deposit may be forfeited as part of that payment of full replacement cost. 5. I agree to care for and clean all loaned equipment in the following manner before returning it to RWALA at the end of the season. I understand that failure to clean equipment may result in a $25 cleaning fee paid for out of my security deposit. a. Do not store equipment in bags when not in use – LET IT BREATHE! b. At the end of the season, shoulder pads, elbow pads, gloves and goalie pads should be hand washed and rinsed (the tub works well) or machine washed IN COLD WATER ON THE DELICATE CYCLE ONLY. They should then be allowed to drip dry for (NO DRYER!) at least 72 hours or until completely dry. c. Completely remove any athletic tape name tag that was placed on the helmet after it was issued (no other stickers should be placed on the helmet), and wipe clean the inside and outside of the helmet with a wet cloth and mild detergent or rubbing alcohol. Wipe down the lacrosse stick in the same fashion. 6. I understand that the equipment will assist in reducing risk of injury to my child, but cannot eliminate or prevent the possibility entirely. I understand and accept the fact that sports participation, including lacrosse, involves risks of injury and bodily harm. These risks are voluntarily and knowingly assumed by myself and my child in connection with his or her use of the loaned equipment.
