Contract - Back side signed by the parent and player/cheerleader • Address must match all forms and utility bill Physical Form - Front side filled out by parent and back side filled out by physician • Physical must be dated 2016 • Signed and stamped with doctor office information (phone number and address) Original Birth Certificate - We must have this for Mt. Baldy Conference to view, copy and stamp (original necessary for certification) • For Returning Players: We may have a copy of birth certificate. Please check with us Report Card - Must have grades from the whole year (3 trimesters or 2 semesters) Current Utility Bill - Electric, Gas, Home phone, Cable or Satellite bills only • No cell phone, car registration, or bank statements will be accepted • Address must match all registration forms • Please note that if you are not a Rancho Cucamonga resident you will need a waiver form signed and approved by the presidents of both cities and conference
Registration fees Must be paid in full by August 1st Football and Cheerleader Fees: Discounts: Jr. Mitey Mites – Midgets $275.00 *Sibling discount $25.00 off Tiny Mites $250.00 *Early bird discount $50.00 off Mandatory League Fundraiser: Discounts: Raffle tickets $125.00 *Early bird buy out $25.00 off Buy out $100.00
Other important Information
Team Sponsorships All teams are responsible to raise $2,500.00 as team sponsorship. These funds cover league costs that registration does not cover. Items like insurance for players, cheerleaders and staff, city fees (player & lights), Pop Warner conference fees, reconditioning and/or replacement of helmets and other equipment, as well as EMT’s and referees. Home Games 50/50 Ticket Sales Each team is also responsible for 50/50 (50% goes to the winner and 50% goes to the league) at all home games. This money is used to help pay the field/score board workers.
Volunteer League Please keep in mind this is a volunteer league and we are doing our best with the children’s best interests at heart. We are all finger printed and all coaches are certified in Heads Up Football. Please try to understand that it takes a lot of time and effort to run this league smoothly. Please, if you are interested in volunteering let us know.
