Release of Liability, Hold Harmless and Indemnity As the parent (or legal guardian) of the above named minor, I have verified that the information on this form/electronic registration of my child is correct and I grant permission for this minor to participate in all of the activities of this sports program. I understand the risks associated with my child participating in football and/or cheer and I assume all risks and hazards incidental to such participation, including risk of serious injury, and do hereby release and waive all claims against Wolverine Youth Football & Cheer, AYF, SDYFC, Westview High School and our clinic coaches and staff, its officers, directors, coaches, sponsors and volunteers and other participants.
