French Broad Baptist Association Bible Camp Permission/Release to Participate
I, undersigned parent or guardian, hereby consent to my child/youth, participating in the F.B.B.A. Bible Camp activities, to include but not limited to, low ropes course team building, swimming, zip line, climbing wall, archery, fishing, all field and gym sports and hiking, at The Oaks Retreat Center. In consideration for being accepted by The Oaks Retreat Center of Greeneville, TN for participation in any activities or events sponsored by The Oaks, I being 18 years of age or older, Hereby Release, Forever Discharge, Agree Not To Sue, and Agree To Hold Harmless, The Oaks and its staff, Board, employees or agents thereof and the French Broad Baptist Association and its representatives, from any and all liability, claims, demands or judgments for personal injury, sickness, death, as well as property damage and any expenses of any nature whatsoever which may be incurred by me and/or my child while participating in any activity or event with The Oaks. By my or my child’s participation in any such activity or event I acknowledge that the participant has my permission to participate and I agree that this Release shall be valid and binding as to all such activities/events participated in, unless I give written revocation of such release. Furthermore, I (and on behalf of my child if applicable), hereby assume all risk or personal injury, sickness, death, damage and expense as a result of participation in recreation, work activities or other activities involved with any such activity/event, further, authorization and permission to furnish any necessary transportation, food and lodging for the participant. Photos/videos of my child or ward may be used by FBBA in any and all promotional material. All Campers must be at the Madison Middle School by 1:30 PM on July 22, 2018. Please pick up your th . child(ren) at Madison Middle School when we return around 12:00 PM on July 27 Bible Camp will be July 22 - July 27, 2018 at The Oaks Retreat in Greenville, TN . The price per Camper is $170.00 and all Registration forms must be turned in no later than July 5, 2018. T-shirts cannot be ordered after July 5, 2018 For insurance purposes, EVERYONE must be registered prior to departure. Campers are not permitted to call home except in case of an emergency, and then only with the Director or Assistant Director’s permission. All Campers must be registered prior to departure. Campers’ registration forms must be signed by a parent/guardian; Campers without properly completed forms will not be permitted to attend. Campers can be registered at the Baptist center during normal working hours (8:30 am - 5:00 PM, Tuesday, Wednesday, and Thursday). Many churches collect their members’ forms & payments and bring them to the Baptist center all together. MEDICAL PROBLEMS/MEDICATIONS: Campers who have medical problems (allergies, illness, etc.) or are taking medications must notify the Camp Nurse. A written explanation of medicines (type, dosage, when to be given, etc.) must be included on the Registration form. All medicines are to be given only by the Camp Nurse so all medications can be properly administered and documented. PLEASE BRING • your Bible, notepad & pen; • 4-5 pairs of casual clothing for church service and 4 – 5 pairs of recreational clothing for activities • a lightweight raincoat or poncho; • pillow and sleeping bag or sheets & blanket; • flashlight and toilet articles such as towels, washcloths, soap, shampoo, toothbrush, toothpaste, etc Dress Code In fitting with the intent of Bible Camp both staff and campers shall exhibit modesty in their dress. The following guidelines are to be followed: 1. Skirts/Dresses for staff/campers shall be no more than 1” above the knee. 2. No ‘short-shorts’ for campers/staff. Shorts are to be 1” below fingerprints. 3. No strapless, halter tops, or spaghetti straps for tops on female staff/campers 4. No muscle shirts or altered T shirts for male staff/campers 5. Girls: Non-white tee shirts and shorts shall be worn over swimsuit Boys: Non – white tee shirts and shorts or long swim trunks shall be worn WRITE YOUR NAME on all towels, underwear, shirts, pants, etc. so that they can be properly claimed & returned. DO NOT BRING electronic devices, food, or clothing not suitable for a Christian Camp. Lost & found items will be held for up to 2 weeks; unclaimed articles will then be donated to a local charity. Call Baptist Center (689-2845) to reclaim lost articles. Bus transportation will be provided from the Madison Middle School. Due to insurance restrictions, all Campers must ride the buses unless they have written permission from their parent/guardian; Campers are not allowed to drive their own vehicles to/from Camp. We have a limited amount of medical insurance coverage for accidents incurred while at Camp. Meals for visitors are $8 per individual Campers are not allowed to have cell phones, I-pads at camp. If campers need to call out they are to go through their counselors or another member of the staff. In case of an emergency the staff will contact the parents/guardians. IMPORTANT INFORMATION PLEASE READ We appreciate you considering our camp. Bible camp has been a positive influence for our youth for 40+ years. A number of youth have come to know Jesus, rededicate their lives, and answer the call to Christian ministry during this week. Bible camp has God’s blessings on it as we have been able to reach youth throughout the area. We carefully select the staff/counselors who work at camp. Our staff/counselors are members in good standing with our local churches and are recommended by their pastors. Due diligence is paid by having background checks performed for all of our staff members. They pay their way to camp as well as attend training to work with our youth for the week. The following are guidelines for Bible Camp: Camp fees will be nonrefundable after the deadline for camp registration (July 5th, 2018) If staff/campers camp fee is being paid by a church the registration forms are to be processed through the church who will then deliver them to the Baptist Center. The church must confirm they will be paying for the staff/campers. Campers are not allowed to have cell phones, I-pads at camp.
