MALLARD CREEK OPTIMIST CLUB Spring 2015 GIRLS SOFTBALL REGISTRATION REGISTRATION MUST BE RECEIVED BY February 21, 2015

1) In person sign ups are: Saturday, January 31, 2015: James Martin Middle School IBM Drive (10am till 12 noon) Saturday, February 7, 2015: Dick's Sporting Goods 8809 JW Clay Blvd (10am till 12 noon) Wednesday, February 11, 2015: Academy Sports 8675 Concord Mills Blvd (6:30pm till 8:30pm) 2) Determine which division your child will play in and input on registration form at item #1. Make up of Softball divisions are subject to PARTICIPATION. The MCOC softball programs follow NFHS and NMFL Fast-pitch rules. Some rules are modified by MCOC. These modifications are posted on our website and provided to all coaches prior to the beginning of the season. Softball Divisions

5) 2015 Extras: Please ensure correct spelling as uniforms will be ordered before skills day Team t-shirts for $12.00 and Team hooded sweatshirts for $22.00 will be avaliable for pre-order only NO LATE ORDERS WILL BE ACCEPTED! 6) Coaching: All coaches must be members of the Mallard Creek Optimist Club. If you wish to become a member, contact us in person, or by email. Coaches are given privilege upon seniority. Background checks are performed on all prospective coaches. MCOC reserves the right to withhold the privilege to coach based on these results or any other criteria they deem pertinent. Only Coaches who fill out form will be allowed on the field during games 7) Concession Stand Opt-out Plan for 2015 Season: Many of you have busy schedules and do not have the time or energy to volunteer for the concession stand each year for each child. MCOC has created this opt-out plan so your name will be added to the "Requirement Fulfilled List". The team parent or coach will not ask you to volunteer for the stand. Cost of this Opt-out plan is $40 to be paid at registration time.

8) The Mallard Creek Optimist Club (MCOC) provides a safe haven for children and complies with all federal, state and local laws, ordinances and regulations pertaining to the protection of children.
