Before requesting a transcript through the counseling center, I must have the Transcript Request Permission Form filled out, signed by my parent/guardian, and returned to our registrar Mrs. Johnson before I can request and pick up my transcript(s). It is then up to me to mail it/them to scholarship committees or college admissions offices. True
