I hereby give permission for my child to attend the Academy for Kids Holiday Program. I fully understand the program has activities and excursions my child will attend therefore give my child permission to participate. In the event of illness or accident i authorise Academy for Kids and nominated authorised emergency contact to obtain medical/ambulance assistance and agree to pay all expenses incurred. I give permission to take photos of my child for the purpose of the program. I give permission for my child to watch PG movies. I give permission to walk on excursion outside the grounds within a 1km radius and commute by bus or other means of transportation to any excursion. I understand the excursion risk management plan.
