Dear Illustrious and Noble Sirs and Ladies, Exclusive Destination Management is pleased to welcome the Southeastern Shrine Association to Jacksonville for your annual meeting in 2014.

Group Dining Options for the SESA Groups

Listed in this proposal are some of Jacksonville’s most popular dining locations located within 5-10 minutes of the Hyatt Regency. Dinner prices listed are estimated to include a 3-course dinner for 15-30 people in a private dining space. SERVICE INCLUSIONS:       SERVICE FEE:

Research and recommendation of restaurants based on extensive experience in Jacksonville Site inspection to approve restaurants based on profile of group Confirm and re-confirm, reservations at restaurants Plan menus and negotiate pricing (where applicable) Advance restaurants and make appropriate introductions Coordinate payment with restaurant and provide itemized billing to client

Charges will be based on actual restaurant charges, plus an EDM management fee of 10% for bar packages or drinks on consumption.

TRANSFER COSTS:

Transportation from the Hyatt to selected restaurant: ....... $5.00 per person

MINIMUM GROUP:

15 people

NOTES:

Menus are to be pre-selected by the group. As these are Private space requests they incur food and beverage minimums that must be met for exclusive use. All restaurant costs will also incur a 7% Florida State Sales tax and a 20% gratuity.

ALL PRICES AND POLICIES ARE SUBJECT TO CHANGE WITHOUT NOTICE.

Pricing in this proposal is based on 2013 rates. There could perhaps be a 5-7% increase for 2014.

The Blue Fish Restaurant & Oyster Bar The Blue Fish offers awesome food and a great atmosphere. Super friendly service, relaxed, cool decor, and very fresh local fish prepared a multitude of ways. In an area of town known for its great restaurants this place does really well to stand out.
