Please Note: Prior to submitting this proposal, one person from your organization’s staff or board must attend an informational session with SVP. Please detail two to four specific projects for which you’d like SVP pro bono consulting during the one-year award period. How can SVP assist in developing, launching, and/or growing your social enterprise? This is one of the most important questions in this form – please be specific.

Attach a short professional bio for each staff member, board member, or volunteer leading your organization’s social enterprise effort. Please include up to five. Attach an organizational chart including staff names. Attach a list of Board Members. Attach proof of IRS federal tax-exempt status, also called a Letter of Determination, dated within the last five years. Attach anti-discrimination or inclusivity statement adopted by the board of directors. Attach all of the following financial documents: organizational budget for the year for which funds are requested; social enterprise budget; most recent financial year-end balance sheet (ie. Dec, 2013); Statements should include budget vs. actual. and, most recent fiscal year-end income statement. Statements should include budget vs. actual.
