I, the parent or guardian of the above named individual, acknowledge that participation in athletic events necessarily involves the risk of physical injury. I further acknowledge that the programs of Faith Lutheran Church’s FaithKids Sports Academy are primarily administered by adults who are trained in first aid. In consideration for accepting the registration of the named individual and permitting the voluntary participation of said individual in its programs. I (for myself as well as for my child, his/her heirs and assigns) hereby release, discharge, and hold harmless Faith Lutheran Church, its employees, volunteers and other representatives or affiliates from and against any claims arising out of or relating to illness, physical injury, death, or other damages that may result to said individual while participating in a FaithKids Sports Academy sponsored event, including any physical injury by the negligence of Faith Lutheran Church, its employees, volunteers and other representatives or affiliates while performing his/her duties during any FaithKids Sports Academy activity. I attest that my child is physically capable to participate in the FaithKids Sports Academy events. However, should employees, representatives or volunteers determine in their sole discretion that completion or participation in any FaithKids Sports Academy program events would be injurious to my child’s health, or should my child become ill or injured, I consent to his or her removal and treatment by any physician or medical care provider at the direction of the event officials, sponsors, representatives and/or volunteers. I give my permission for free use of my child’s name and picture in any participation of FaithKids Sports Academy in future publicity for FaithKids Sports Academy.

Mission Statement: Offer physical and spiritual growth for kids, through a quality sports program, that teaches different sports and exercises through innovative techniques that will allow kids to learn, play and have fun in a Christian environment.
Registration Form One Registration Form per Class/per Child
Sports Sampler (Ages 3—6) Kids will learn the fundamentals of many different sports in this high energy, fun-filled, class. Soccer, football, basketball, and floor hockey are just a few of the sports we’ll focus on. The emphasis will be on team work, effort, cooperation, and our most important friend, Jesus. Please wear tennis shoes and bring a water bottle. Little Kickers Soccer Camp (Ages 6-8 & Grade 35) Kids will love this introduction to soccer. The fundamentals of kicking, dribbling, shooting and goaliework will be taught through fun and innovative techniques. Teamwork, cooperation, and effort will be emphasized! Kids will also get a chance to scrimmage. Please bring a water bottle and wear tennis shoes. Kids Tee-Ball Camp (Ages 4-6) Kids will learn the primary skills of hitting, running, fielding and throwing. The players will gain an understanding of the fundamental rules by playing games, which allows them to get prepared for league play. Emphasis is on having fun, teamwork and effort. Please bring a water bottle and wear tennis shoes. Gloves optional! Basketball Camp (Grades 3-5 & 5-7): For beginning and intermediate players. Using progressive drills and games, players will learn basketball and soccer dribbling, passing, positions, shooting techniques, offenses and defenses. Kids will scrimmage during class time. Emphasis is on effort and teamwork. Please wear tennis shoes for basketball camp and soccer cleats and shin guards for soccer camp. Bring a water bottle.
