**Student Athletes coming directly from practices, please shower first! All students should dress in shorts and short sleeves** ** All proceeds go to the SHS Sports Medicine program for the purchase of medical supplies. Avoid the long lines! Please pick up forms from the SHS athletic office or print from school WEB Page – Athletics Tab – sport physical and clearance paperwork--fill out in advance. Forms will also be available the evening of physicals.
EXPRESS LINE: Between May 1st and May 16th drop off completed forms and payment to the Athletic Office. Completed forms will be at Physical Night and when you arrive you will go straight into the physician line. Sport Physical and Orange Cards Information: 1. Fill out all five(5) Sports Physical Forms (EL2, EL3, EL3CH, EL3VCHA) and ORANGE CARD (also known as the emergency card) 2. Sports Physical Forms/Documentation will expire after one calendar year (365 days). 3. If you did not start the 9th grade at SHS, please obtain a GA4 Form from Athletics Office. 4. Eligibility is based upon an Unweighted GPA of 2.0. 5. Athletic Office will clear the athlete for try-outs or participation in sport at SHS. 6. After all forms are reviewed and GPA checked; an ORANGE CARD will be certified. 7. If an athlete participates in multiple sports, an ORANGE TICKET will be issued for each season.
