II. Payment Information: Registration Fee: $65 (Insurance coverage, Player Cards, and ISFC Club Fees) Discounts for multiple players per family: 3 rd child’s registration fee is waived Notice: Each team's fees will be different depending on age and number of tournaments attended
III. Waiver: I hereby for myself, my minor child, my administrators, heirs and assigns waive and release any and all right and claims for damages, including negligence, I may have against the Indiana Southwest Futbol Club, Inc., its board of directors, officers, commissioners, chairpersons, referees, and/or coaches, for property damage or personal injury, including death that may arise or grow out of my minor child’s participation in ISFC’s Soccer Program. I also agree to indemnify and hold harmless the Indiana Southwest Futbol Club, Inc.; its board of directors, officers, commissioners, chairpersons, referees, and/or coaches any claim, damages, expenses or action, including attorney fees, assessed or brought against them because of my acts or omissions or the acts or omissions of my minor child. I give ISFC my permission to use photographic images of my child or website and/or advertising purposes.

Team Placement Guidelines Fall 2016 – Spring 2017 2010 & 2009 Birth Years Fall 2016 and Spring 2017 Seasons play 8 and under 4v4 (no GK); ISL will not accept requests for 2011’s to join.
2008 & 2007 Birth Years Fall 2016 and Spring 2017 Seasons play 10 & under 7v7 ISL will not allow a 2009 team to play in this group. 2006 & 2005 Birth Years Fall 2016 and Spring 2017 Seasons play 11 & under (2006’s) and 12 & under (2005’s) 9v9 ISL will not allow a 2007 team to play in the 2006 (11 & under) group. 2004, 2003 & 2002 Birth Years Fall 2016 and Spring 2017 Seasons play 13 & under (2004’s), 14 & under (2003’s), and 15 & under (2002’s) 11v11 ISL will not allow a 2005 team to play in the 2004 (13 & under) group. Fall Only: 2003 (14 & under) will be allowed a maximum of 3 trapped players per game (2003’s, 15 & under) 2001, 2000, 1999, 1998 & 1997 Birth Years Spring 2017 Season Only Play 16 & under (2001’s), 17 & under (2000’s), 18 & under (1999’s), and 19 & under (1998’s) 11v11 Unlimited number of 1998’s may roster with the 1999’s Will we have enough teams for a separate 1998 (19 & under) Division? Or do we need to merge the 1998/1999’s into one age group? 1997’s would join the 1998’s.
Additional ISL Programming Options Fall 2016 2002 & under (15 & under) and 2001 & older (16 & older) 7v7 division Clubs will be able to merge players as needed for this season to form a team
