2013-14 USA HOCKEY TRANSFER & STANDARD PLAYER RELEASE FORM CANADIAN ADULT FEMALE ONLY USA Hockey regulations require all non-US citizens to reside in the US and process a transfer to USA Hockey to complete the registration process. There is no fee for female player transfers. All transfers should be processed at the start of the playing season.

Residency Section Players must provide proof of residency. Acceptable residency documents are: Alien Registration Receipt Card or Permanent Resident Card or one of the following visa types: A, E, F, G, H, I, J, L, M, O, P, Q, R or TN. For further information regarding classification and citizenship eligibility, please see the USA Hockey Annual Guide, II Classifications, C. Youth, Girls/Women, (2) Citizenship Eligibility. A player in the US on a B1/B2 Visitor Visa is not eligible to register on an adult female team. Release Section Release section only needs to be processed if player is transferring from one federation to another. Not required on renewal transfers.
