Pre-Registration is required Team Registration Night Friday, September 22nd – 6:00-7:30pm At Connell Rink 220 Broad St, Weymouth Email [email protected] for the Team Application and Player Consent forms to be brought to registration night
Divisions for Mites through Bantams $250 Per Team of up to 10 Players (No Individual Registrations)
Cost Includes T-Shirt & Pizza for all, Championship Game per Division and winning T-Shirt Bake Sale Table and Raffles

Teams must register for the division based on the oldest birth year on the team. No player is allowed to play down in a division but a younger player may play up Youth XL, Adult Small, Medium, Large, X-Large

$250 Registration Fee –Checks made payable to Weymouth High Hockey Boosters • Each player needs to provide their own Hockey Helmet, Stick and Gloves • Each Team needs to provide their own set of goalie equipment • Any inappropriate behavior can result in an individual or team being removed from the tournament without a refund

My son/daughter has my permission to participate in the Street Hockey Tournament Fundraiser sponsored by the Weymouth High School Boys and Girls Teams. I assume responsibility in case of accident or injury. By my signature below, I/we grant consent to any/all health care providers designated by the fundraising staff, to provide my child with any necessary medical care as a result of any injury/illness. I hereby for myself, my heirs, executers and administrators, waive and release any and all rights and claims for damages I may have against Weymouth Public Schools, and any and all associated with this event including but not limited to the Town of Weymouth, the Athletic Department, sponsors, coordinating groups, individuals and volunteers, and will hold them harmless for any and all injuries suffered in connection with this event. I attest and certify that I am physically fit to compete in this event. Further, I hereby grant full permission to any and all of the foregoing to use my likeness in all media including photographs, or any other record of this event for legitimate purposes.
