I   hereby  release  the   District  117   Board  of   Education  and   its   employees  from  all   claims  on   account  of   any  injuries  which  may  be  sustained  by  my  child.  I  agree  to  indemnify  the  Board  of  Education  and  its   employees  from  any   claim,  which  may  hereafter  be   presented  to   our   child  as   a   result  of   any  injuries.  If   medical  attention  is  required  for  injury  or  illness  while  in  camp,  I  give  permission  for  such  medical   care.   Participants  should  wear   athletic  type   clothing  and   tennis  shoes.  Hair   must  be  pulled  back   and   off   the   shoulders.  NO  JEWELRY;  NO  GUM.  Please  send  your  child  with  plenty  of  water  and  a  snack  if  you  wish.   Parent  or  Guardian  Signature:

Mail  or  drop  off  Registration  Form  and  Payment  to:     Athletic  Office,  Antioch  Community  High  School   1133  Main  St.  Antioch,  IL.  60002   Attention:  Matthew  Dawes     Registration  must  be  received  by  June  1,  2015.    Any  registration  received  after  June  1st  will  NOT  be  guaranteed  a  T-­‐shirt.
