Name of Participant:__________________________________ I am signing this Release so that I can participate in RC Softball Clinic to be held 7/10/2017 . This Release, Indemnification, and Assumption of Risk Statement covers all events and occurrences associated with the Activity. I understand that if I have concerns about my health or ability to participate, it is my responsibility to discuss my concerns with my physician before deciding to participate in the Activity. I acknowledge and understand the risks inherent with the activities carried under this program. I agree to assume the risk that unexpected events may occur and result in loss, harm, injury, or illness to me or damage to my property while I am participating in or observing the Activity or while I am traveling to or from the Activity. I hereby agree to indemnify and hold harmless Ripon College , Ripon Softball, its sponsors, employees, volunteers, affiliates, officers, agents, successors and assigns, subordinates, and any other persons connected to this event from any liability. In the event that I require emergency medical treatment, I give my permission for evaluation, diagnoses, treatment, and/or medication in accordance with the standard medical practice by licensed medical personnel. I relieve Ripon College, and Ripon Softball of all responsibility and consequences that may arise as a result of treatment. Further, I agree to accept any and all financial responsibility as a result of the performed treatment. I consent to the provision of emergency medical treatment to the extent that the treatment is necessary in the medical opinion of the doctor rendering treatment.

PROSPECT DAY On July 10th, 2017 the Ripon College softball program will be hosting a Summer Prospect Day, a one-day clinic for grades nine through twelve. Athletes will be under the direction of the head softball coach, Brooke Kalman. The goal of this clinic is to educate future student-athletes and provide campers with the opportunity to enhance their skills. If the student athlete is interested in a tour with admissions please indicate this on your registration form. STAFF: Additional staff members will include current Red Hawk coaches, as well as former and current Ripon College players. All of our staff members are dedicated to providing a positive experience and are committed to the development of young players. FOCUS: - Hitting - Fielding - Pitching - Catching - Game Play - Evaluation
DATE: July 10th, 2017 GRADES: 9th- 12th LOCATION: Ripon College/ Tracy Field (on campus) DETAILS: - Check in begins at 7:30 a.m. (at the softball field) - Fall Prospect Day begins at 8:15 a.m. - Fall Prospect Day will conclude at 3:15 p.m. COST: $100 - Prices includes T-Shirt - Checks can be made payable to : Ripon College (write Ripon Softball in the memo)
