2 Locations: Pasadena High School/Rose City High School PUSD is proud to offer the following summer school options for high school students. Classes will be filled on a first come first serve basis. Courses will be taught using direct instruction, blended learning, and online methods. Please see the courses offered and times below and complete the Summer School Enrollment Form.
Biology/Chemistry PHS REMEDIATION ONLY
Offered as 3- hour sessions of direct instruction

SUMMER 2015 ENROLLMENT FORM (Student must be enrolled in PUSD for 2015-2016) 1. Review summer school class offerings with counselor’s assistance/review. 2. Applications may be submitted to CIS Academy located at 2925 East Sierra Madre Boulevard, Pasadena, CA 91107,Office P107. CIS is located on East Washington Blvd next to the Pasadena High School Campus. Office hours are from 8:004:00pm. Applications will be time stamped on the day they are received.
3. Schedules will be available to pick up from school counselors by June 1. 4. Classes will be filled on a first come first serve basis. Each school has been allocated a percentage of seats based on their 2015 total enrollment. 5. Transportation will not be provided by the district except for students who participate in the Special Education Program

SUMMER SCHOOL 2015 BEHAVIOR/ATTENDANCE CONTRACT All students enrolled in the summer school program will comply with the PUSD standards of student conduct. We expect your son/daughter to attend regularly and display acceptable behavior. Summer school is a privilege and a voluntary choice. Students who fail to comply with behavior and/or attendance standards may be withdrawn at any time. Students who fail to show for classes on the first day may be dropped and their spot filled. 3 tardies counts as 1 absence; no more than 3 absences are allowed in this program. PLEASE LABEL YOUR (1) FIRST, (2) SECOND, AND (3) ALTERNATE CHOICE IN THE BOX TO THE LEFT SIDE OF THE DESIRED COURSE AND TIME. STUDENTS MAY TAKE BOTH DAY AND EVENING COURSES.
