I have given my child permission to participate in the Tiger Volleyball Camp. I authorize the camp instructors to act for me according to their best judgment in any medical emergency. Despite the use of school district facilities, this opportunity is sponsored and managed fully by Bethany Trimble and the Tiger Volleyball Camp and not by USD265, Goddard Kansas. I acknowledge that despite the use of Goddard School District facilities, the district is not sponsoring or managing this camp and I release USD 265 and its employees from any and all liability in case of any injury during the event.
