To all Travel Team Coaches: For over 50 years Concord Youth Hockey (“CYH”) has been fortunate to have volunteers dedicate countless hours of time developing hockey players and helping to make their youth hockey experience a positive one. Thank you for volunteering your time with CYA. As with any organized sport, there are some important forms that need to be completed prior to the beginning of the season, as well as forms that will be distributed during the season. In order to keep our teams in check with the Granite State League, the Valley League, the Seacoast League, and USA Hockey, it is important for players/parents to complete the forms as soon as possible and it is the Head Coach’s responsibility to see that his or her team is compliant. To start the season, each team will be given a team binder that outlines the information to be compliant and will serve to store all the necessary information. You should bring this notebook to all CYH events noting this book will contain consent to treat forms that might be necessary in the case of an emergency once you collect them. In addition to the consent to treat forms this binder will contain information required at NH state tournaments and it is recommended you bring to all games and team events. Below are is a list of forms you will need for the season the majority of which are accessible on the Concord Youth Hockey website. The Head Coach will receive a listing of players, guardians, and their contact information from Theresa Leppard, CYH Registrar. It is suggested that the Head Coach send an e-mail of introduction providing directions to the parents to print out the necessary forms from the CYH website (they are available on the “Resources” page, have them filled out, and brought to the first team practice with direction that if all forms are not complete that they will not be allowed on the ice. This letter of introduction should also make clear that each player is to register with USA Hockey prior to the first practice and that the fee to do so is in addition to the CYH tuition. Please request that the USA Hockey registration be forwarded to the Head Coach as these forms must be turned into Theresa in order to produce a team’s roster. Without being properly registered with USA Hockey, a player or coach is not allowed to participate in any on-ice activities.

All forms need be filed with the binder in alphabetical order by player last name. Please organize your notebook in the following manner: q

Credential Sheet; You will receive this from Theresa Leppard, CYHA Registrar. Please do NOT fill it out, this form is for the State Registra when they review your book for credentialing. This should be on the first tab of your binder. Signed Roster: Once you return all USA Hockey registrations to Theresa Leppard, Theresa will coordinate producing and getting to you your roster. Once received, it is necessary that each player and coach sign the roster to then be returned to Theresa who will coordinate having the signed roster certified (signed) by the State Registra. Thereafter, the certified roster is to remain in your coaching book for review at the Granite State Tournament. Note unlike in years past, we are NOT keeping copy of birth certificates in our coaching book. Your USA Hockey roster will indicate in column “V” which players have and have not been verified. Those who have a “V” in said column are all set. Those absent a “V” will require you to obtain an copy of the players birth certificate to be provided to Theresa who will coordinate having it verified by the State Registra who will update the roster as verified “V”.

Code of Conduct: Players and coaches are required to sign/date this form. Blank forms are available on the CYHA website in the Coaches Resources page. Please date, sign, and file in your notebook. Consent to Treat: Parents/guardians and coaches are required to complete this form. Blank forms have been placed in your notebook. Please date, sign, and file in your notebook. NHAHA SafeSport Code of Conduct Form: All Coaching staff are required to sign/date this form. Blank forms are available on the CYHA website in the Coaches Resources page. Please date, sign, and file in your notebook. Youth Transfer Forms: To be kept for our Canadian born residents. (Need passport, Permanent Resident Card, Birth Certificate to submit to USAH – only form goes in book). Coach Card: Copy both front and back of card for head coach and each assistant. (Online USAH online certification list) Game Sheets: Game sheets should be filed in date order in your notebook. Note game labels are to be in Alpha order- not by jersey number. Note: See section regarding game sheets below for mailing original game sheets. 2

IMPORTANT: You need to submit the names of your assistants, their birth dates, and patch numbers to Theresa Leppard. Theresa will add the assistant coaches to your roster. Your players and coaches must sign this roster for the Granite State League. Please return this signed roster to Theresa ASAP! CYH needs to submit the signed roster to the district registrar. Games played without the signed roster on file will not count. If you add players to your roster, contact Theresa as soon as possible. Theresa will provide you with a supplemental roster for signature. Once the district registrar signs this roster, Theresa will return a copy for you to keep in your notebook. As with your regular roster, games played without a signed supplemental roster do not count. If you have provided your coaching patch number in the past, your number should be on file. If you have not provided this information, please forward your patch number (CEP # as listed on your card) to Theresa Leppard. Please make a copy of your coaching card (both sides of your card) as well as copies of your assistant coaches cards and keep these copies in your notebook. You will need this information to qualify for state tournaments. Note: Please remind your assistants that CYH will reimburse them for the cost of the coaching class once completed. You will need a copy of your receipt.

Valley League: If your team is playing in the Valley League, note that due to the number of teams, it is nearly impossible to obtain a game swap within the Valley. It is imperative that you coordinate with Steve Norton in as much advance notice as possible (prior to the publishing of the Valley Schedule) any specific times and dates that may be in conflict for your team. A game forfeit due to our not showing up for a game within the Valley costs CYHA several hundred dollars per incident. Missing games because you want to do a tournament instead, or because the game is “too early” or “too late” is not an acceptable option. Games missed due to inclement weather are obviously understandable.

Player Release Form: If any of your players played for another hockey program last season, we are required to keep a release form on file for that player. The parents/guardians of the player need to obtain a signed Player Release Form from their previous organization. This signed release form must be kept in your notebook at the back of the book. 3

Parents/guardians of your players can go to the Resources page on the CYHA website and print out a blank form. The completed form needs to be returned to Theresa Leppard so she can forward the form to the GSL. q

Game Sheet Labels: A game sheet label template is available on the CYHA website on the Coaches Resources page. If you cannot print labels, as soon as you know your players jersey numbers, email this information to John Drew with the numbers. John will print game labels for you to use on your game sheets. These are basic labels and should have your coaching numbers on them. The GSL and the Valley league require Labels on every game sheet. Labels will be placed in your notebook. John will have a supply of Avery Labels #5163 available if you need extra labels. Note: CYH receives a $10.00 fine each time game labels are not used on game sheets. Game Sheets And Score Reporting: You will be provided with blank game sheets for your GSL games. Game sheets must be mailed to the NHAHA/GSL Divisional VPs within 72 hours following the day the game was played. The name and address of the Divisional VP is located on the inside cover of your notebook. Note: If the Game Report Sheet is not received within that period, an automatic fine of $25.00 will be levied against the Association of the hosting team. All game scores must be recorded on the CYHA website Each coach will have access to the team’s game schedule on the CYHA website and can add scores. You can also use the SportNgin mobile application to modify game scores. Instructions on how to do this will be on the Coaches Resources page on the CYHA website. Contact John Drew if you have difficulty with the process. All Granite State League (GSL) game scores must be recorded on the CYH website no later than Sunday night each week, even for games played Sunday evening. In turn, one of the CYHA board members will enter the score information into the GSL system before midnight on Monday. If game scores are not reported on time, an automatic fine of $25.00 (per game) will be levied against CYH. Seacoast Hockey League (SHL) game scores must be recorded on the CYH website no later than Sunday night each week, even for games played Sunday evening. In turn, one of the CYHA board members will enter the score information into the SHL system before midnight on Monday. The name and address of the Seacoast League contact person is located on the inside cover of your notebook.

How to submit game information to the Concord Monitor: All of the players like to see game reports in the roundup, and our sponsors donate with the expectation that their name will be advertised as much as possible. Coaches or their designee must report all games in the sports roundup section of the Concord Monitor to include reference to the team sponsor (i.e. The “RiverStone” Concord Squirt 1 Team) by logging onto ConcordMonitor.com at the roundup submission page at the following address: http://drupal.concordmonitor.com/form/submit-asports-roundup. There is also an instruction document on the Coaches Resources page on the CYHA website. Note that CYH depends on sponsors to keep the costs as low as possible and therefore it is a requirement that each week team’s scores be submitted. This is not an option.

Volunteers: It is also your responsibility to find volunteers to run the clock and keep the game sheet during the games. Please find reliable parents to do this on a regular basis. Any volunteers that have contact with players in the locker room must undergo background checks and SafeSport training. Volunteers that are only handling the clock and game sheets do not fall under this category. Note: Please remind volunteers to remain impartial during the game. They are not allowed to yell, cheer, complain to the refs, ring cowbells, etc., during the game.

Player Tournament Eligibility: Players must play 50% of games to qualify for the state tournament play. If a player is injured, please note that on the game sheets so the state can track the 10/20 rule. Players who play “up” for a higher level team (i.e. a “2” team player playing on the “1” team) to help fill in may only play a limited number of games with the higher level team before they are permanently rostered on that team. If you wish to call a player up, please check with Tim Donlon on the requirements. Players can not play “down” to a lower level team.

Rink Directions: Directions to the rinks should be included in the location URL on the game schedule. Rink directions can be found on the NH Hockey website as well as the Valley website. Coaching Coordinator: Please contact Dunc Walsh, Coaching Coordinator, with any questions regarding drills, or practice conduct 5

questions. Tim Donlon can be of assistance as well and is your resource for coaching patch questions. q

Referee In Chief: Please contact Joe Mielcarz, Referee In Chief, with any referee issues, or any ruling questions. Joe is also responsible for scheduling referees for all home games. Ice Scheduler: Please contact Wes Riley, Ice Scheduler, with all practice schedule questions and issues. Travel Team Scheduler: Please contact Steve Norton, Travel Team scheduler, with any game scheduling questions. Steve is responsible for scheduling all GSL and Seacoast League Games. If your team moves up or down a tier based on early season performance, Steve will reschedule your GSL games. Don Crandlemire is your contact for Valley League and Seacoast scheduling. Note: If you are playing in 2 leagues, there are bound to be a few game conflicts during the season. Please coordinate and plan accordingly.
