This season’s uniforms will be High 5 Wildcard Two-button jersey #312201 (youth) and #312200 (adult) and Alleson Athletic belted pant. You will need to supply me with color choices on pant, jersey, belt, and socks. Remember that color choices are a first to return basis, which means that the first completed sheet I receive wanting red jersey and black pants will be the team in that division to wear those colors. There will be no repeat colors in any division. Be aware that Colors of jerseys, socks, and belts might vary from each other slightly. I will do my best to make you aware of any big differences so you can decide accordingly. All ladies in the system prior to evaluations were notified to try on uniforms at evaluations. You are responsible for getting your new teams info onto this form!
Forms are due to me by Monday August 21st at 9 PM Failure to send your form will result in standard sizes and numbers ordered for your team. This is required to get all uniforms printed before our first game. Preferred method to send your order for is to save the PDF document with all information and email to [email protected]. Any orders with names to be printed on jerseys must use this method. Please see instructions on final page for emailing this document. If you do not have names to be printed you may scan and email or hand deliver. Uniform order forms must be legible if you choose this method. DO NOT take a picture of it and text it! Only order as many uniforms as girls you have been assigned. Remember that it is each parents’ responsibility to try on uniforms to ensure proper fit. If there is a manufacturing defect, it can be replaced, but we are not responsible if you get the sizes ordered and they don’t fit. Coaches may order matching jerseys. This is a unisex cut shirt so order accordingly. Coach jerseys are $20 each including printing. Individual names can be put on back of jersey at $5 per name. Money must be in my hands by August 21st at 9 PM along with your order. Payments may be taken from sponsor funds. Please copy the league treasurer on your uniform email if you wish to use sponsor funds. Please don’t hesitate to call or email me with any questions or concerns.
