Adult Protective Services Investigation and assessment of reports for adults at risk of being mistreated, neglected, or exploited and are unable to protect themselves Caregiver Relief / Day Respite Care Assistance to help with the care of older adults who need 24-hour supervision Case Management Ongoing assessment, consultation and service planning with seniors and their families in the home setting Community Meal Sites Provides a nutritious meal and an opportunity for socialization Emergency Needs Assistance Emergency assistance to purchase medication or utilities Emergency Response Buttons / Automatic Pill Dispensers Access to emergency services through electronic communication devices and assistance with medication compliance

Medication Management Medication monitoring and management by a registered nurse Personal Care Assistance in the performance of necessary activities, such as personal hygiene and grooming Personal Needs Pantry Provides personal needs items PRIMETIME Senior Services monthly newsletter Project Lifesaver A tracking system for those at risk to wander, such as clients with dementia, operated by the Union County Sheriff’s Office Project Safe Heat Provides furnace cleaning and inspection and free carbon monoxide detectors Senior Centers Community and Seasoned Citizens (CASC), Richwood Civic Center, and Windsor and Community Seniors, offering social activities and opportunities for community involvement Transportation Wheelchair accessible transportation for medical appointments, community meal sites, banking and shopping Welcome Bags for Seniors
2016 Service Numbers In Home Services: 252 seniors received over 12,600 hours of service Homemaking Services: 174 seniors for average 2-4 hours per week Personal care: 59 seniors for 2-10 hours per week Respite care (in home): 11 seniors for 3-10 hours per week Respite care (in care facility): 1 Skilled Nursing: 9 seniors for 4 hours per week Emergency Response Button Devices: 233 Medication Dispensers: 16 Transportation: 192 seniors were transported for a total of 4,974 trips to medical appointments, shopping and banking Meals subsidized by Senior Tax Levy: 44,778 home delivered meals to 354 individuals 35,948 meals at congregate meal sites to 1012 individuals 3,567 weekend home delivered meals to 48 individuals Adult Protective Services (APS) referrals investigated: 54
