Course Repeat Request This form must be completed by the student-athlete in the event they need to repeat a course for their academic program in order to continue to meet progress toward degree standards for athletic eligibility. This form should also be completed if a student-athlete is unsure they are registered for a repeat course. In order for this repeat course request to be considered, this form must be completed in its entirety. Incomplete forms will not be reviewed. It shall be the student-athlete’s responsibility to complete this form. This form must be submitted to the Compliance Office upon completion.
