COST: $65 (Includes a camp t-shirt) LOCATION: Westlake High School CAMP DATES: JUNE 13TH: 8am - 11am JUNE 14TH: 8am - 11am JUNE 15TH: 8am - 11am PARTICIPANTS: Players who are going to be in 4th to 8th grade in 16-17 School Year STAFF: The camp will be taught by the Westlake High School coaching staff as well as the Varsity, JV, and Freshman players. Please complete the registration form below and return to WHS, attn.: Ms. Mental at WHS by April 9th, 2016. A $10 late fee will be added for all late registrations.

Medical Release: All Players should have a sports physical. I give my daughter _______________________permission to participate in the Westlake Volleyball Camp. I understand that participation in volleyball, as in any athletic activity, involves a risk of injury. Therefore, Westlake Volleyball Camp, the instructors, Westlake High School, and its employees will not be held liable for any accident or injury that might occur under normal playing conditions at the camp.
