Saturday, August 6, 2016, 9AM to 5PM (lunch provided) Sunday, August 7, 2016, 2:30PM to 4PM WHCC Gymnasium 25 minutes per game (10 minutes for each half and 5 minutes for halftime) Saturday, July 23, 2016 - Individual sign-up at http://goo.gl/forms/c4j694pdMeYc195Z2 (link also available at www.whcchome.org or via QR Code) - If unable to register online, please submit paper registration form to Joe Jong in person or via email at [email protected] $5 per player (Free for kids) - Signed Waiver and Release form is required to participate in the tournament - For player under 18 years of age, signature by parent or guardian is required WHCC members, visitors, friends and participants of WHCC activities Medals for 1st and 2nd places in each division July 24, 2016, 1pm (Tournament committee will assign a captain for each team and draft players into specific teams)

WHCC 2016 Summer Basketball Tournament Waiver and Release Form I hereby release West Houston Chinese Church (WHCC), its pastors and governing councils, and volunteer staff from any liability whatsoever arising out of my injury, damage, or loss that may be sustained by said person during the basketball tournament held in WHCC’s gym. In the event of an accident, I give the WHCC staff members and volunteers the permission to secure medical aid, and I relieve WHCC of liability in such an event. I also agree to bear all incurred medical expenses. I understand that this release becomes effective from the date I signed the form, and remains effective until the end of the WHCC basketball tournament. If the participant is under 18 years of age, a co-signature of a parent of a legal guardian is needed.
