PARENTAL CONSENT: I, ____________________________, desire for my child, __________________________, to participate as a ball boy/girl of East Central University (ECU) (the “Activity”) and do hereby give my consent to allow my child to participate in the Activity. I certify that my child is physically and mentally able to participate in the Activity and that my child will follow all appropriate safety precautions and instructions. I am aware that ECU will not be liable for injuries that may occur during participation. I therefore voluntarily agree to assume all risks and responsibilities in any way associated with this Activity.

In consideration of and return for services, facilities, and other assistance provided to me by ECU in this Activity, I release ECU, its governing board, officers, agents, employees, and any students acting as employees (the “Releasees”), from and against any and all liability, claims, demands and actions that may arise from injury or harm to me or my child in connection with this Activity. I understand that this Release covers liability, claims and actions caused entirely or in part by any acts or failures to act of ECU, including but not limited to negligence, mistake, or failure to supervise by ECU. I acknowledge that ECU does not carry life, health, or accident insurance on my child. ECU has advised me that I will be responsible for any medical charges which my child may incur if my child is injured during the Activity and that ECU has encouraged me to secure my own such insurance. THIS GENERAL RELEASE WAIVER OF LIABILITY IS ALSO BINDING ON THE STUDENT’S AND/OR THEIR PARENTS’/ LEGAL GUARDIAN’S PERSONAL REPRESENTATIVES, HEIRS, AND ASSIGNS. THIS IS A RELEASE OF LEGAL RIGHTS. READ AND BE CERTAIN YOU UNDERSTAND IT BEFORE SIGNING.
